Booking Policy
New patients are required to complete their intake/health history form BEFORE their appointment. If you opt to complete the intake form when you arrive, please note that no additional time will be added to your scheduled treatment.

Confidentiality
When you book your first appointment, you will be required to complete an intake health history form. Please note that information requested will assist me in treating you safely.
Please note that all information provided in the intake form will be kept confidential unless allowed or required by law. Your written permission will be required to release any information.
If your health status changes in the future, please let me know. This form will be updated yearly.
Feel free to ask any questions about the information being requested, at any time.

STRICT 24-hour Cancellation Policy
This policy is strictly adhered to - meaning it applies to your 2nd no-show or cancellation.
If you cancel or reschedule your appointment with LESS than 24-hours notice, for any reason, you will be charged a 50% cost of the appointment. An email will be sent to you requesting payment of the fee, if it is not paid the cancellation fee WILL be added to the cost of your next treatment.
Exception: The cancellation fee will only be waived IF you fill your spot.
If you are 15mins late for your appointment, your appointment will be cancelled.
Appointment Times
The time you booked is for the duration of the appointment not the table time/ as some may put it "hands - on" time.
If you are new patient, I recommend, booking 60-min appointment. The first 15mins of your appointment is spent reviewing details on your intake form, the assessment of the area of concerns and the treatment plan for the appointment, with you.

Refusal of Service
For safety reasons, I reserve the right to refuse or discontinue treatment at any time, which includes but are not limited to the following:

violence or threatening behavior

intoxication
-
theft
-
any violation of policies

sexual or romantic advances
-
illness or infectious conditions
-
contagious or unidentified skin conditions (e.g. rash, warts, bug bits, etc.)
If you develop a skin condition, please clear it with your doctor before receiving a massage. If you exhibit any of the abovementioned conditions or behaviors, you will be denied treatment and charged 50% of the cost of the booked appointment.
